About the OHT shared space

In partnership with Ontario Health, RISE launched an online shared space for OHTs to exchange resources and lessons learned, participate in discussions, and collectively solve problems related to the implementation of the OHT model. The shared space hosts both targeted, time-limited collaboratives as well as communities of practice. 

RISE is supporting three collaboratives focused on using a population-health management approach to ‘move the needle’ on equity centred quadruple-aim metrics for three priority populations:

  • older adults and/or people with chronic conditions
  • people with mental health and addictions issues
  • people who could benefit from a palliative approach to care.

Ontario Health and/or other health system partners are supporting four communities of practice that relate directly to two of the eight OHT building blocks to help OHTs as they progress and mature.
