Top Ten insights into improving hospital-to-home transitions: Part 1 - older adults with complex health and social needs

Older adults with complex health and social needs experience frequent transitions (moves) from hospital to home. Transitioning between hospital and home after a hospital admission can be a stressful and confusing time for patients and their caregivers. François-Pierre Gauvin, Maureen Markle-Reid, Carrie McAiney and Rebecca Ganann shared insights into how to improve hospital to home transitions that were gained from: 1) an OSSU-funded pragmatic randomized controlled trial of a nurse-led transitional care intervention for older adults with multimorbidity and depressive symptoms,  2) a citizen brief, and 3) recent Citizen Panel on how to engage patients and caregivers in transitional care.
