As communicated through the September 2023 OHT Acceleration announcement, and in alignment with Your Health, Primary Care Networks will be an important part of OHT acceleration. This webinar will provide an overview of the Primary Care Network (PCN) in OHT Guidance. During this webinar, we will hear from family physicians, primary care clinicians, and system leaders such as Dr. Liz Muggah, Dr. Catherine Yu, Dr. Briana Yee-Providence and Sarah Hobbs, who will share key insights and observations on the value of establishing PCNs in OHTs.
